These forms are REQUIRED for Journeys of Compassion. We must receive your Application Form and Confidential Reference Form along with your $350 CAD deposit before you will be accepted to the team. Please read over the Liability Release Form and Team Unity Agreement Form CAREFULLY, as YOU WILL BE REQUIRED TO SIGN THEM THE FIRST DAY OF THE JOURNEY. PLEASE DO NOT PURCHASE NON-REFUNDABLE AIR TRAVEL TICKETS UNTIL YOU RECEIVE NOTIFICATION OF ACCEPTANCE FROM IMPACT NATIONS. |
